DESCRIPTION:

Care co-ordinators sit at the centre of a person’s care, enabling them to achieve more effective and efficient care by understanding what matters to patients, their social interactions and their clinical requirements at the same time.

The Personalised Care Institute mandate that all care co-ordinators must attend a minimum of a 2-day care co-ordinator course and, where possible, also understand key aspects of Personalised Care and Support Planning and Shared Decision Making.

Our two-day live virtual, highly practical and interactive programme covers the core professional competencies, knowledge, skills and behaviours set out in the PCI Curriculum and NHSE Workforce Development Framework for Care Coordinators. It focuses on the core communication, relationship buildingskills and continuous development, essential to personalised care and extends these to enable participants toapply them into their own context.

Each live day consists of facilitated training, delivered by two experienced health and wellbeing coaches. Theprogramme content includes:

  • Understanding personalised care and the role of a Care Coordinator
  • Understanding different communication models and how to apply them in conversations
  • How to have personalised care conversations
  • Understanding what matters to a person
  • Demonstrate active listening
  • Ensure the effective flow of information and facilitate partnership working
  • Able to quickly assess someone’s needs and build trust

The programme incorporates:

  • whole group discussions
  • small group, multi-professional and peer learning
  • pairs work
  • live practise of conversations and use of tools and approaches
  • individual and small group practice
  • underpinning models and theories
  • demonstrations
  • working with case studies, examples of real patients and client scenarios and working through approaches
  • reflective learning
  • supervision and mentoring

Between training days, participants are encouraged to undertake work-based learning, self-directed learning,reflective practice and gather multi-source feedback.

All course slides and handouts are provided to participants and a workbook containing core models and frameworks is issued to assist learning and reflective practice.

Day 1 Introduction to

Care Co-ordination

  • Further understand and build your confidence in your role as a Care Co-ordinator
  • Learn what personalised care is and how to deliver it in time- pressured environments
  • A framework and new tools in how to have better personalised and empowering conversations
  • How to feel lighter at the end of the day, looking after and managing yourself and your emotions even better
  • The powerful difference between the art and act of listening and skills to do this
  • How to engage people that may be disengaged or stuck by asking powerful questions to understand what matters and is important to them
  • All embedded with practise and feedback from your peers and our
    trainers

Day 2

Care Co-ordination Fundamentals

• How to build effective relationships and networks

• Explore what a good care plan looks like

• Understand patient activation and motivation

• How to; deliver personalised care on the phone, create shared goals

and outcomes, build confidence and motivation in others to act, using

appreciation and feedback

• How to use a powerful tool to help people who are stuck or don’t

know what they want to move forwards

• All this along with lots of practise to embed these new skills

 

CPD Certificate

HCA will provide you with a CPD certificate for the training hours you

have completed. For this programme the CPD hours will be 12.
