Improving health equity for people with Learning Disabilities, Autism, and Severe Mental Illness
North East London is one of two national sites piloting a Combined Health Check (CHC) for individuals with Learning Disabilities (LD), Autism, and Severe Mental Illness (SMI). This innovative approach brings together elements of the three existing annual health checks into a single, person-centred process — reducing duplication and supporting more integrated, personalised care.
The pilot is being delivered across selected GP practices in NEL between July and December 2025, with evaluation running through to March 2026.
