Pathfinder is primarily a PHM analytics tool that can support the delivery of a population health management (PHM) cycle – from identifying population cohorts, to monitoring interventions, and evaluating outcomes. It leverages NEL linked data from both primary, community and secondary care to name a few, offering a comprehensive view that enables more informed decision-making and collaborative working across the system.
