What is Pathfinder?
NEL are working in partnership with Optum on using their Pathfinder platform, which is a PHM analytics tool that can support delivering the PHM cycle – from identifying population cohorts, to monitoring interventions, and evaluating outcomes. It leverages NEL linked data from both primary, community and secondary care to name a few, offering a comprehensive view that enables more informed decision-making and collaborative working across the system.

Understanding the platform’s capabilities will enable VCFSE partners to engage with its insights via your local partners).
