SUMMARY:Support for embedding the three personalised care roles session
DESCRIPTION:CLICK HERE TO REGISTER \nThis session is for all who have a role in managing\, embedding and working alongside the personalised care roles in primary care for example PCN managers\, Social prescribing Managers\, GPs\, ICS colleagues. \nThe session is held by the Social Prescribing and Community Based Prevention Team and TPHC and we’ll be joined by Jonathan Sampson from NHSE London Primary Care Team. \n  \nDuring the session\, you will: \n– Learn about the latest key resources\, guidance and top takeaways for supporting these roles (Workforce development frameworks\, toolkit\, ARRs funding and GP contracts) \n– Connect with others across London also managing/embedding the roles to share challenges and ideas \n– Help us understand what regional support is useful around managing/embedding the personalised care roles
