DESCRIPTION:What is the NHS Type 2 Diabetes Path to Remission Programme? \nThis is a 12 month treatment programme with the aim of achieving diabetes remission. The programme is lifestyle-led health management\, rather than a medication first approach. It supports patients with significant weight loss (15kg)\, improvement in HbA1c and reduction in medication needs. Patients are offered a choice of digital or F2F care and their Oviva clinician supports them in 1:1 sessions through two phases: \n\nOviva Change – 12 weeks of total diet replacement\, 800-900 calories a day. DESCRIPTION:The DES requirement for 2022-23 was that “by 30th September 2022\, as part of a broader social prescribing service a PCN and commissioner must jointly work with stakeholders including local authority commissioners\, VCSE partners and local clinical leaders\, to design\, agree and put in place a targeted program to proactively offer and improve access to social prescribing to an identified cohort with unmet needs. This plan must take into account views of people with lived experience.” The NEL Personalised Care team would be very keen to hear how you got on and would like to invite you to share and reflect on the outcomes of your projects through a set of one-hour learning sets.
