The meeting will begin with a presentation about Well Newham and then we will have presentations from one person representing each personalised care role before moving into discussion groups. \nAGENDA\n1:00pm – Katy Szita and Augusta Adu\, Co-chairs welcome and share survey\n1:05pm – Ysabella Hawkings\, LBN Public Health Principal to present about Well Newham as a tool to support residents and how it can support personalised care roles\n1:20pm – Social Prescriber presentation\n1:30pm – Health & Wellbeing Coach presentation\n1:40pm – Care Coordinator presentation\n1:50pm – Community Neighbourhood Link Worker presentation\n2:00pm – Community Connector presentation\n2:10pm – Care Navigator presentation\n2:20 – 2:40pm – Break into groups to discuss key questions\nReflective practice\, peer support\, team meetings and supervision\nWhat can the roles learn from each other re reflective practice\, supervision\, peer support and should/could we make more space for peer support as a Community of Practice?\nWhat are the barriers we and residents/patients face to resolving their support needs and is there a way our roles can work more collaboratively to address this?\nDiscussion and agree actions to survey feedback.\n2:40 – 3:00pm – Feedback from group discussions\n3:00pm – Meeting close \nWHAT WILL YOU DO IN THE FORUM?\nLearning\nNetworking\nSharing good practice\nThis is a chance to meet and talk to various One Newham organisations\, find out what resources and training are available to support you in your role and build your community of practice together. \nTARGET AUDIENCE\nSocial prescribers\, community link workers\, care navigators\, ELFT community connectors and anyone who has elements of prescribing in their role or are interested in the role.
