June 10 @ 12:30 pm - 1:30 pm

Yorkshire Health Partners have been delivering CYP Social Prescribing services since 2021. We have experienced the pitfalls, success and developed a CYP SP Service Blueprint from inception, ideas through to full-service development. We developed the service from GP referrals only to including self-referrals, schools and colleges and Voluntary and Community Groups.

Our service is data informed; voice of service users and partners, commissioner engagement and driven by robust and meaningful data analyses.

As the Health landscape evolves, developing prevent, out of hospital care and locally led partnerships CYP Social Prescribing is a great example of a community service in action.

We invite you to join us on the 10th June from 12.30 to 13.30. Bring your lunch, bring your questions, and let’s explore together!

Introduction

Establishing a CYP Social Prescribing team can profoundly enhance the well-being, development and resilience for children and young people. Early intervention is a proven tool to prevention and mitigates crisis management, can reduce CAMHs waiting lists and lengthy waits for support where mental health conditions can be exacerbated.

CYP Social Prescribing employs a holistic approach to address the social, emotional, and practical needs of the younger population, providing them with personalised support and connections to community resources. By fostering resilience and improving mental health, the CYP Social Prescribing team can empower children and young people to thrive across many spheres of their lives, education, family, personal and relationships. Building a foundation for long-term success and prosperity for children and young people.
