Social Prescribing Evaluation Toolkit Launch

We are pleased to share with you our NEW Social Prescribing Evaluation Toolkit.

The co-produced toolkit is designed to guide local services in demonstrating the impact of their services.

It includes a wealth of local examples across London of how impact has been demonstrated, and what enabled them to get there, including:

  • North-East London’s approach to social prescribing impact – developing a clinical template for social prescribers and an ICS wide dashboard
  • Barnet social prescribing service quarterly and annual reporting combining GP, social prescribing and VCSE data to inform commissioning
  • North-West London’s approach to monitoring and recording social prescribing data through Joy
  • Local evaluations of social prescribing services in Merton and Bromley
  • And more…
